Requires the signature of the Course Instructor and Academic Advisor (this form is not for students PERMing into classes or dropping out of classes within the “Add/Drop” period). |

Requires the signature of both Course Instructors if seeking to take two courses that overlap in times. If the overlap exceeds 15 minutes, requires the Dean of Faculty’s signature. |
