The Registrar’s Office supports the academic mission of Scripps College by preserving the integrity, accuracy, and privacy of all academic records; interpreting academic policies and procedures for members of the Scripps community; and distributing these records effectively and efficiently within applicable policies and regulations.

General

Link	Description and Notes
Change of Address Form	If a student’s permanent off-campus home address has changed.
Change of Advisor Form	To change major advisor, complete with the signature of student’s current and potential advisor. Submit completed form to the Office of the Registrar ([email protected]).

Major and Minor Declaration Forms

Link	Description and Notes
Major Declaration Form	Requires signatures of Major Advisor and Academic Advisor (if not the same) before submission.
Off-Campus Major Declaration Form	Requires signatures of Major Advisor (at other institution) and Academic Advisor (at Scripps) before submission.
Minor Declaration Form	Requires signatures of Major Advisor and Minor Advisor (if not the same) before submission.
Course Change Form for Major or Minor	For students who need to edit their major/minor declaration form. Requires signatures of Major/Minor Advisor and Academic Advisor (if not the same) before submission.
Preliminary Major/Minor Declaration Form	FOR FIRST YEAR STUDENTS ONLY. Requires the signature of Academic Advisor before submission.
2020-2021 Senior Thesis Registration Form for Non-Science Majors	Senior Thesis Reader / Information Form
Senior Major/Minor Form Upload	Requires Major Declaration and Minor Declaration (if applicable) forms to be completed and uploaded.

Changes in Registration/Enrollment

Link	Description and Notes
Change in Course Registration	Requires the signature of the Course Instructor and Academic Advisor (this form is not for students PERMing into classes or dropping out of classes within the “Add/Drop” period).
Course Conflict Form	Requires the signature of both Course Instructors if seeking to take two courses that overlap in times. If the overlap exceeds 15 minutes, requires the Dean of Faculty’s signature.
Course Withdrawal Form	Requires the signature of Course Instructor and Academic Advisor and must be submitted on the last day of instruction before Reading Days.

Academic Petitions

Committee on Academic Review	The catch-all form for all students seeking exceptions to academic policies (study abroad, transcripts, course credits from other institutions, etc).
Race and Ethnic Studies GE Requirement	Requires the signature and statement of Course Instructor/Department Chair and Academic Advisor before submission.
Gender and Women’s Studies GE Requirement	Requires the signature and statement of Course Instructor/Department Chair and Academic Advisor before submission.
Internship Credit	Requires signatures of Supervising Professor and Academic Advisor before submission.
Independent Study	Requires signatures of Supervising Professor and Academic Advisor before submission.
Early Graduation	Requires statement from faculty members involved as well as Academic Advisor(s) before submission.
Plan/Petition to Participate in Commencement Prior to Completion of Degree Requirements	The information a student is required to provide changes depending on the student’s major. Read the instructions closely before submission.
